FormsPlease click on a link below for the appropriate form:First Response Course Roster Course Monitoring Form Course Request Form Course Evaluation Form Instructions for filling out and submitting a roster:Click on a form name to open it. Please note that you must have the free Adobe Reader software (a download link is below) to open these forms. You may print a blank form to be filled out by hand or fill it out in Adobe Reader and print it as a completed form (except for the instructor signature). Figure 1 below illustrates the print buttons to be used in printing a form (some forms may have only one of these buttons, not both). Whether blank or already filled in, simply click one of these buttons to print the form (you may need to pick the printer you want to use, as with printing anything else from your PC).To fill out the form in Adobe Reader: Click on the first field, shaded in blue, that you want to fill and type in the appropriate information. You can move to the next field by pressing the TAB button on your keyboard (or press SHIFT + TAB to move backwards a field). To fill in the check boxes on the left side of the form, click on a box to check it (click again to remove the check). You can use TAB to move from field to field and SPACE BAR to check or uncheck the selected box but it's faster to simply click the boxes. Don't forget to fill out page 2 of the form in the same way as page 1. Click the first field on the second page to begin filling in student information. If you get "lost" at any point, simply click on the field you want to edit and the cursor will move there. When you're finished simply print the form. Advanced Tip: You can save a roster on your computer (including any information you already filled in) and then open it later to finish it or print it. In figure 1 below, the second button, just to the right of the print button, is the save button. If in doubt, move your cursor over the button and a description of the button will pop up automatically. If you fill out this roster frequently, you can save a little time by filling out your information and saving the form so that all you have to add is the course and student information and you have a completed roster, in less time!
